Request a Flood or Low Flow Discharge Form

Discharge Request 

Authorized by PA 451 of 1994. Completion of this form is voluntary.

Important:

  • A confirmation e-mail will be sent to you.  If you do not receive it, please e-mail your form information directly to deq-lwm-qreq@michigan.gov?subject=Discharge Request .  You must click the Submit button at the end of the form to send the request to us and get a confirmation e-mail.
  • Please limit an attachment's file size as much as possible.  Our incoming e-mail is limited to 15 MB by the state's email handling system.  Form submittals larger than this will still generate a confirmation e-mail to you, but we will not get your form submittal e-mail.

  • Low flow discharges are typically only needed if you are applying for an NPDES permit, a mixing zone determination, or a dam impoundment drawdown. If this is the case, please indicate your DEQ program contact person.

Location* - Describe with as much detail as possible; e.g. Westernmost Trib to Smith Creek at private bridge, 1000 feet upstream of Smith Road, in the SW 1/4 of Section 4, T4N, R6E, Smith Township, Smith County.

Please remember to provide a site location map. This should be a photocopy of the USGS quadrangle map or a county map with the site clearly marked. You can include it as an attachment (below), mail it to the above address, fax it to (517) 241-0275 (please note Discharge Request on the cover sheet), or email  it separately as an attachment.  If you are submitting several discharge requests at the same time that use the same map, please only supply the map once.
