The Michigan Commission for the Blind (MCB) Central Administrative Office and the Library Service for the Blind and Physically Handicapped are both in Lansing. The residential MCB Training Center is in Kalamazoo. MCB Regional Offices are located in Detroit, Escanaba, Flint, Gaylord, Grand Rapids, Kalamazoo, Lansing, and Saginaw. • Eligibility

• MCB Eye Report Form 
Use this form to refer eligible blind and visually impaired patients age 14 and over to the Michigan Commission for the Blind. This form may be filled out electronically and then printed for doctor and patient signatures, or the form may be printed blank and filled out and signed by hand. Please fax or mail the completed form to the MCB central office in Lansing or your local MCB office.
