• Doing even more for your low-vision patients... Eye doctors are often the first professionals that people consult when they experience vision loss. After doing all that you can do professionally, you can do even more -- by referring your eligible low-vision patients to the Bureau of Services for Blind Persons.

• Eligibility The Bureau of Services for Blind Persons provides services to blind and visually impaired Michigan residents of all ages. These services are provided using state and federal funds. Read more about eligibility for services...

• BSBP Eye Report Form PDF icon Use this form to refer eligible blind and visually impaired patients age 14 and over to the Bureau of Services for Blind Persons. This form may be filled out electronically and then printed for doctor and patient signatures, or the form may be printed blank and filled out and signed by hand. Please fax or mail the completed form to the Bureau central office in Lansing or your local Bureau office. This form was updated October 1, 2012, with the name of the new bureau (the Bureau of Services for Blind Persons) created by Governor Snyder's Executive Order 2012-10.
