The department established the Michigan Diabetes Outreach Networks (MDONs) to ensure comprehensive diabetes management for persons with diabetes residing in Michigan. The mission of MDONs is to reduce the burden of diabetes on the individual, the family, the community, and the health care system. This is accomplished by each MDON developing collaborative partnerships with health care delivery agencies, sponsoring and providing professional education, and coordinating and developing diabetes resources within their service region. Such collaboration of diabetes care resources and attendant increased awareness that diabetes is a serious disease help to ensure that persons with and at risk for diabetes are identified, entered into the health care system, and receive ongoing preventive care and education.

Each of the Michigan Diabetes Outreach Networks champion diabetes statewide by:

• identifying diabetes care, education, and support resources;
• establishing a regional advisory council to facilitate collaboration and provide advice on planning, operation, and evaluation;
• developing collaborative partnerships with health care delivery agencies and providers to facilitate the delivery of diabetes care and education, according to current clinical recommendations, research, and standards;
• sponsoring and providing professional education and public awareness activities; and,
• collecting, analyzing and reporting data on the outcomes of MDON and partner agency services.
