Ambulatory Care Sensitive hospitalizations are those for which timely and effective ambulatory care can help reduce the risks for common problems such as asthma, diabetes or dehydration. High hospitalization rates of such conditions in a community may indicate either a lack or failure of prevention efforts, a primary care resource shortage, poor performance of primary health care delivery systems, or other factors that create barriers to obtaining timely and effective care.

periods 1991-2004 for preventable hospitalizations in your community. The
information can then be viewed for the state, or any of Michigan's 83 counties
or 44 local health departments.
