ENROLLMENT

New Employees and their Dependents

You can enroll yourself and your eligible dependents before or within 31 days after your eligibility date.

If you do not enroll when newly hired, or you do not enroll your eligible dependents at that time (or when newly acquired), you will be given other opportunities to enroll or add your dependents, but only during Open Enrollment periods, as long as eligibility is maintained.

Effective Date of Coverage

Employees

Your coverage is effective on the first day of the first payroll period on which you are eligible and which begins after you register to enroll.

An eligible employee who is not enrolled but is covered by the enrollment of a spouse or parent may enroll before or within 31 days after termination of the spouse or parent’s coverage. The effective date of coverage is the first day of the payroll period after the date of termination or after enrollment, whichever is later.

Dependents

The effective date of coverage for your existing enrolled dependents is the same date as yours.

Enrollment of New Dependents

If you wish to enroll a newly acquired dependent after your coverage becomes effective, you must notify your human resources office in writing within 31 days of acquiring this new dependent. The coverage effective date for a newly acquired dependent will be the date he or she is acquired (by birth, adoption, marriage, divorce, etc.).

The effective date of any change in any premium contribution that may be required will be the first day of the payroll period following the date a new enrollment form is completed.

Open Enrollment Period

During an announced Open Enrollment period, employees who are not already enrolled may register to enroll. Also, employees who are already enrolled may change their enrollment to include eligible dependents. However, in order for an employee to enroll as described, he or she must be actively at work on the date of registration. The effective date of enrollment or change in enrollment will be as prescribed in the Open Enrollment materials.

Employee Transfer to Another State Agency or Bargaining Unit

If you transfer from one State agency to another, your existing enrollment will be automatically transferred to the new agency without interruption. If you transfer from one bargaining unit to another, your enrollment will be automatically transferred and will reflect changes in certain collectively bargained benefits or premium contributions, if any.

Death of Employee

In the event of your death, State Vision Plan coverages will automatically continue for your dependent if he or she will receive a monthly pension benefit from the State of Michigan.

If your dependent(s) is not going to receive a pension benefit following your death, coverage for your enrolled family will end 30 days following your death unless your dependent continues State Vision Plan coverages under the Continued Group Insurance System (CGIS) by making direct, full premium payments (including the share of the premium that was paid by the State). See the NOTE under the “Termination of Dependent Coverages” section.

Identification Cards

Once you have enrolled, you will receive in the mail within 45 days a State Vision Plan (BCBSM-printed) identification card.  If you are also enrolled in the State Health Plan administered by BCBSM, you will receive one ID card that can be used for both your State Health Plan and State Vision Plan benefits.

Your human resources office can verify your State Vision Plan enrollment during the first 30 days of coverage. Coverage can be verified by BCBSM after that time.

Employees will also automatically receive within 45 days a new State Vision Plan identification card whenever there is a transfer to another bargaining unit.

If you lose your State Vision Plan identification card or if you wish to receive an additional card, you may call the BCBSM State of Michigan Customer Service Center (toll-free) at 1-800-643-4652.

If you want to change your mailing address or if you wish to change or cancel your State Vision Plan identification card or enrollment, you must contact your human resources office.  The BCBSM State of Michigan Customer Service Center is not authorized to change your enrollment record information.
