Replacing a Lost or Stolen State ID Card - In State
If your standard or enhanced state ID card is lost or stolen, you may visit any Secretary of State office to apply for a replacement. You will need to present identification to substantiate your identity. A new photograph will be taken. A lost or stolen enhanced ID card is immediately canceled and cannot be used to cross the border.
Your replacement ID card will be valid for up to four years and expires on your
birthday in the year shown under the expiration date.
You will be
required to provide proof of identity
and your
Social Security number.
Acceptable identity
documents for the standard and enhanced ID cards vary, but some of the documents
that are accepted for either include a valid, unexpired passport; certified birth
certificate with a raised seal issued by a U.S. or U.S. territory government
office; federal or Michigan government issued photo ID card or a valid U.S.
military photo ID card. More than one document may be required - faxes and
photocopies are not accepted. Please review to the

More than one document
may be required - faxes and photocopies are not accepted. All Social Security
information will be verified.
If you are replacing
a standard ID card and are ineligible for a Social Security number, you must
present a letter of ineligibility from the Social Security Administration and
proof of legal presence.
If your enhanced state ID
card is lost or stolen and you also need to make a change in your name or date
of birth, you must visit a
Secretary of State office authorized to issue an enhanced state ID card. A
lost or stolen enhanced ID card is immediately canceled and cannot be used to
cross the border.
Replacing a Lost or Stolen State ID Card - Out of State
If you are traveling in the U.S. and lose your standard or enhanced state ID card, contact the Department of State Out-of-State Resident Services Unit at (517) 322-1473 to request a replacement. You will need to fill out an application with your first, middle and last name, date of birth, state ID card number and out-of-state address. Return the application to:

Standard ID Card: Contact the Out-of-State Resident Services Unit for assistance in obtaining a duplicate ID card.
Enhanced ID Card: Contact the U.S. Embassy in the country you are visiting or the U.S. Customs and Border Patrol when you arrive at your U.S. port. The enhanced ID card is one of the federally accepted travel documents for re-entering the U.S. by land or sea from Canada, Mexico, Bermuda and the Caribbean.

Name Changes
If you need to change
your name on your valid state ID card you must present it with proof of the name
change. Proof of your name change must be established through an official
record, such as a certified marriage license, divorce decree or a legal name
change document. A fee will be charged to correct your name and a new
photograph will be taken.
